Be the difference.Job SummaryProvides psychosocial evaluations, psychotherapeutic interventions, case management and discharge planning services for patients and/or their families. Provides and acts as a liaison for physician, patient and/or family between the hospital, community agencies and facilities.Essential FunctionsAll team members are expected to be knowledgeable and compliant with Prisma Health's purpose: Inspire health. Be the difference.Performs intake assessment/evaluation of family dynamics and treatment needs, including creation of treatment plan.Prepares case history of patient's personal, social, and emotional development.Assists patients to evaluate both their strengths and symptoms, and facilitate patients setting their own goals, and plan for appropriate services.Provides therapeutic intervention with patient family including individual, marital, parental, family and group therapies.Interprets nature of illness and prescribed course of treatment to patient and families. Communicates family issues to other staff members.Helps patients develop social skills, interests, and leisure time activities to provide a sense of participation and personal worth, including opportunities for age appropriate, culturally appropriate activities.Assists patients to identify appropriate employment opportunities and vocational rehabilitation services. Functions as Designated Examiner which involves the commitment process, designated exam, provide testimony for probate courts.Performs other duties as assigned.Supervisory/Management ResponsibilitiesThis is a non-management job that will report to a supervisor, manager, director or executive.Minimum RequirementsEducation - Master's degree in Social Work or Counseling, Psychology or related field of study.Experience - Two (2) years post-graduate behavioral health/counseling experience.
