Prisma Health - - Responsibilities: Provide personal care and comfort services to terminally ill patients in home or Cottingham Hospice House as assigned and supervised by the registered nurse; Assist with ambulation, transfers, and activities of daily living to promote patient safety and comfort; Measure and record vital signs and report changes to RN promptly; Perform basic housekeeping duties to promote hygiene, safety and comfort for the patient; Participate in team conferences and care planning to discuss patient and caregiver needs
