Job Title: Transition Liaison

Qualifications:

  1. Possess an associate's degree from an accredited college or have earned 60 semester hours (or equivalent) from an accredited college;
  2. Possess basic computer skills and working knowledge of applications (Excel, Google, Word, Email);
  3. Demonstrate skill proficiency in oral and written communication and organization;
  4. Have knowledge, understanding, and acceptance of students with disabilities in high school settings;
  5. Ability to work productively and independently while managing multiple locations, deadlines, tasks and time;
  6. Ability to relate and work well with students, teachers, administration, faculty and staff, parents, businesses/employers, and community partners;
  7. Experience with job development, coaching, and placement;
  8. Understanding of data collection systems and reporting;
  9. Adhere to both department and position guidelines and procedures;
  10. Ability to handle unforeseen situations in accordance with policy;
  11. Ability to respect and maintain confidentiality;
  12. Ability to take initiative in the job;
Reports To: District Transition Coordinator

Supervises: NIA

Job Goal: The job of the Special Education Transition Liaison (TL) is to assist in facilitating and preparing high school students with disabilities for successful transitions between school and post-secondary education, training, and employment. They support the development, implementation, and monitoring of school-based and community-based work training experiences and related functional skills training programs.

The TL works collaboratively with students, faculty and staff, parents, businesses, service agencies, and adult service providers, to promote and support the employment of students with disabilities.

Performance Responsibilities:
  1. Knowledge of district, state, and federal special education policies and procedures specifically related to transition services and post-secondary outcomes for students with disabilities.
  2. Contact and provide information to the business community to establish, maintain, and monitor training and employment sites for students with disabilities.
  3. Coordinate with the high school/career center Work-Based Learning (WBL) teams and ensure proper documentation, evaluation procedures, and submission requirements are adhered to for WBL and College & Career Ready (CCR) experiences and hours.
  4. Assist teachers and support staff to determine and identify career exploration and work-based learning experiences for students in alignment with post-school goals.
  5. Work with classroom teacher(s) to develop and execute introductory career awareness and career exploration activities for students
  6. Monitor student performance in a variety of training and employment sites; visit job sites, provide coaching and support as needed to students and employers/work supervisors, work with employer to complete evaluations, serve as a liaison between the student and employer for the purpose of identifying areas requiring additional training or accommodations to master skills and bolster independence on the job.
  7. Assist with transition and/or work-based learning assessment/work evaluation of students at the request of district personnel or employers.
  8. Foster collaborative relationships with Special Education personnel, schools, parents, students, businesses, agencies, and other entities in planning work-based learning and transition services related to the students' Individual Education Program (IEP).
  9. Maintain consistent spreadsheet entries and updates regarding assigned school, student, and transition program data and submit within designated timelines.
  10. Demonstrate knowledge of District policies, procedures and regulations.
  11. Other duties deemed necessary by supervisor.

Terms of Employment: 9 ½ months with salary in Group H with added benefits.

Evaluation: Performance of this job will be evaluated in accordance with the provision of Board Policy.
