Job Summary
Provides support in planning, directing and coordinating care of patients. Assists in obtaining patient medical records, schedule patient diagnostic testing, contact patients by phone and/or written correspondence. Writes letters, mail and fax documents. Develops and maintains organized records and documentation.Essential Functions

Requests and obtains outside medical records from various providers and/or facilities
Schedules initial and follow up appointments.
Contact patients by phone, MyChart, written correspondence regarding appointments
Faxes or mails medical information to case managers, referring physicians, dialysis units and consulting physicians
Performs other duties as assigned.
Supervisory/Management Responsibilities
This is a non-management job that will report to a supervisor, manager, director, or executive.
Minimum Requirements
Education - High School diploma or equivalent. Associate degree preferred
Experience - Two (2) years related experience
