Job Summary
Works in collaboration with the multidisciplinary care team to assess and coordinate chronic disease management for patients with chronic pulmonary diseases. Initiates and accepts patient referrals, assesses healthcare needs, identifies community and medical resources, and manages assigned patient population. Works with coordinator and navigators across the system to assure standardized care and follow-up.Essential Functions

Develops and implements patient-centered care plans and/or discharge pathways to support self-management of pulmonary disease and to prevent readmission. Provides patient and family education as needed regarding chronic lung conditions.
Participates in multidisciplinary rounds and teams to assist in identification of patients needing Respiratory support and education.
Identifies and coordinates post-discharge services, follow-up needs, and education in collaboration with the multidisciplinary care team. Communicates with home care companies and SNF as needed to help facilitate discharge and education.
Assists with data collection for tracking LOS and readmission and identification of barriers to successful transitional care. -
Performs all respiratory care duties as within the Prisma Health Respiratory Care Policy and Procedure Manual and as assigned by the manager. May be asked to perform alternate duties as needed to assure quality care.
Performs other duties as assigned.
Supervisory/Management Responsibilities
This is a non-management job that will report to a supervisor, manager, director or executive.
Minimum Requirements
Education - Associate degree in Respiratory Care. Bachelor's degree in respiratory care preferred.
Experience - No experience required.

Required Certifications, Registrations, Licenses
RRT - Registered by the National Board of Respiratory Care
RCP - Licensed Respiratory Care Practitioner in the state in which work is performed.
Credentialed as a COPD educator and Smoking Cessation Counselor via the American Association for Respiratory Care (AARC) course will be required within one (1) year of hire.
Certified Asthma Educator preferred.
Knowledge, Skills and Abilities
Experience with multi-disciplinary teams and/or leadership experience preferred.
