Job Details

Serve as the primary nursing contact and care coordinator for patients in pediatric orthopedic specialty clinics. Collaborate with referring and outside providers to coordinate multidisciplinary care plans. Facilitate communication between patients, families, specialists, and external providers to ensure timely follow-up and continuity of care. Work closely with the nurse practitioner to assist with in-clinic procedures, including Baclofen trials, Baclofen pump refills, and Botox/ Dysport injections. Provide clinical oversight, guidance, and collaboration with the International Coordinator as needed to ensure appropriate coordination of care for international patients. Participate in patient education, counseling, and support related to treatment plans and ongoing care. Coordinate clinic scheduling, documentation, and data tracking for specialty programs.
