Job Details

Facilitates the referral of patients from a variety of settings- acute care, post-acute services, physicians, private payors and patients/families. Provides screening, assessment, planning, and coordination of services for the department. Maintains knowledge of federal and state rules and regulations governing home care to patients as well as individual private payor requirements. Excellent communication skills contribute to positive customer service and coordination of care. Works closely with the referral team to provide right time/right services patient care.
