DESCRIPTION:
The purpose of this position is to develop, build and maintain census, by establishing an effective referral program. Build community relationships with medical offices and hospitals. Verify insurance authorizations and payor sources before and upon admissions. Effectively manage the admission agreement process. Implement an organized and accurate record keeping system for patient admission, discharge, and bed hold. Gather needed patient information before, during and after admission.
QUALIFICATIONS:
Successful management of census and inventory in long term care. Proven skills of leadership: communication, and knowledge of local, state, and federal regulations. Ability to relate to residents, families, visitors, clients, and staff.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
· Gather all pertinent pre-admission data to determine patient level of care and the ability of the facility to properly care for the patient.
· Interview potential resident and resident representative to collect accurate data.
· Complete pre-admission and admission inquiry forms accurately
· Interact with discharge planners in an effective, efficient manner. Ask for appropriate and adequate resident information.
· Analyze the information to determine if the facility can care for the residents adequately.
· Must have experience with Point Click Care, Well Sky-Care Port, Availity systems
· Perform all duties as assigned in an effective manner.
· Follow admission, transfer and discharge policies and procedures.
· Maintain an accurate facility census for all facility departments.
· Keep accurate records of patient information. Keep all residents' information confidential.
· Notify appropriate facility departments when a resident is admitted, transferred, discharged, or moved
· Work cooperatively with all facility department supervisors and administrative staff; communicate facility philosophy, policies, and procedures to potential and new patient representatives.
· Explain the facility admission agreement and all attachments to potential and new patient representatives.
· Type and prepare patient identification bands, bracelets, and other appropriate identifying labels.
· Act as a liaison between transferring institutions and the facility. Establish a good rapport with transferring institutions.
· Participate in marketing the facility and maintaining an acceptable census level.
· Refer facility and patient concerns to appropriate individuals.
· Perform other related duties as directed by his/her supervisor.
· Analyze Quality Measure information related to admissions, discharges, and resident case to develop and implement the marketing program.
· Assume Quality Assurance duties as assigned.
· Consistently work cooperatively with patients and their representatives, facility staff, physicians' consultants. and ancillary service providers.
EDUCATION AND/OR EXPERIENCE:
· Company Required: Bachelor's Degree in related field and or 3-5 years in leadership experience in related field or Licensed Nurse.
· Continuing education re: care and management of the elderly and disabled persons or the operation and maintenance related topics.
· Successful leadership and marketing skills required. Must be able to handle conflict, problems, and emergency situations.
· Related skills are dedication to the well-being of seniors, excellent written and oral communication skills, good computer skills such as spreadsheet, Word documents and Power Point, high energy level and the ability to handle multiple priorities is a must.
